U.S. workplace safety regulators have cited a chemical and colorant manufacturer’s Georgia facility for fall and other hazards and proposed $256,088 in penalties.

The U.S. Occupational Safety and Health Administration cited Neenah, Wisconsin-based First Source Worldwide L.L.C. for willful violations for failing to install a fall protection system and develop and implement a written permit-required confined-space program following an inspection of the company’s Dalton, Georgia facility, according to an agency statement on Wednesday.

The company was also cited for several serious and other-than-serious violations, including failing to develop safety procedures when performing equipment maintenance and servicing, failing to train and evaluate forklift operators, failing to ensure employees have proper personal protective equipment, and failing to install machine guards on equipment.

“The hazards identified put workers at risk of serious injury,” Christi Griffin, OSHA Atlanta area office director, said in the statement. “Employers must follow required safety practices to protect worker safety and health.”

A company spokesperson could not be immediately reached for comment.
