In-home care led to big drop in hospital admissions, readmissions: Study

An in-home care program helped Independent Health Benefits Corp. reduce preventable hospital admissions, readmissions and emergency room visits, reports Becker’s Payer Issues. The Buffalo, New York-based nonprofit health plan launched the Care for You program in late 2021. The program, which has 2,200 Medicare Advantage members with complex, chronic conditions, reduced preventable hospital admissions by 54% and readmissions by 45%.
