Florida insurer faces class action for fake health insurance

The Tampa-based insurer has been accused of developing the limited benefit indemnity and medical discount plans, which Simple Health Plans LLC and Nationwide Health Advisors sold to consumers as PPO plans compliant with requirements of the Affordable Care Act.
