Chemical safety board probing refinery explosion

The U.S. Chemical Safety and Hazard Investigation Board is deploying a four-person team to investigate an explosion and fire at a Philadelphia refinery complex.

A fire ripped through the Pennsylvania oil refinery on early Friday — 11 days after another fire broke out at the same facility, according to Reuters.

A spokeswoman for the U.S. Occupational Safety and Health Administration earlier told Business Insurance that the agency was investigating the incident at the Philadelphia Energy Solutions site.

CSB investigations look into all aspects of chemical incidents, including physical causes such as equipment failure as well as inadequacies in regulations, industry standards and safety management systems, the board said in a statement on Friday.
