OSHA issues $171,169 in fines after crane kills worker

The U.S. Occupational Safety and Health Administration has proposed penalties of $171,169 for Crystal Finishing Systems Inc. following the agency's investigation into an employee’s death in June 2016.

Schofield, Wisconsin-based Crystal Finishing specializes in aluminum extrusion and applies powder coatings to a variety of underlying materials.

The death of a 51-year-old chemical technician at the coatings company's facility in Mosinee, Wisconsin prompted the investigation, OSHA said in a statement on Monday. The worker suffered fatal injuries when an automated crane pinned him between a crane hook and dip tank load bars as it moved product to different tanks on an anodizing line. The employee was pronounced dead at the scene, OSHA said.

OSHA said it found Crystal Finishing failed to guard machines to prevent workers from encountering operating parts, protect workers walking working surfaces, provide adequate personal protective equipment, and train workers about hazardous chemicals in use at the facility.

Crystal Finishing could not immediately be reached for comment.
